ALL FORMS MUST BE COMPLETED BEFORE YOU ARRIVE AT PHYSICALS.  YOU WILL NOT BE ALLOWED TO SIGN IN

IF FORM IS NOT COMPLETE – NO PHYSICAL - NO EXCEPTION!!!!

It is strongly encouraged for your parent/guardian to create a profile on the SportsWare Parent Portal. Instructions are located in your sport’s Google Classroom.

ALL STUDENTS PARTICIPATING IN WINTER SPORTS MUST HAVE A PHYSICAL BEFORE PRACTICE STARTS ON (3/8/2021)

If you played or tried out for a FALL and/or WINTER sport,

contact the Athletic Trainer via email for the Blue Recertification form.
